Innovation requires failure, but we hide evidence of setbacks and iterations, so we lose critical learning opportunities and develop risk-averse cultures.

A Fail Wall is a prominent, permanent space for documenting failures and their lessons. Use a simple, consistent format that captures what was attempted, what happened, and what was learned. Include physical artifacts when possible. The wall should feel both serious (these are important lessons) and playful (failure is part of innovation).
